Trauma and coagulopathy: a new paradigm to consider.
نویسنده
چکیده
I njury, intentional and unintentional, is one of the main causes of death for adult Americans. Further, from 1999 to 2003, injury was the main cause of premature mortality (measured as potential years of life lost), ahead of malignancy and heart disease, for Americans who died before their 65th birthday. Injuries that cause massive hemorrhage are often associated with the highest mortality rates. More than 50% of patients who present with massive hemorrhage die, and for those who die within hours of the injury event, it is often the most common cause of death.

Acute traumatic coagulopathy and trauma-induced coagulopathy: an overview
Hemorrhage is the most important contributing factor of acute-phase mortality in trauma patients. Previously, traumatologists and investigators identified iatrogenic and resuscitation-associated causes of coagulopathic bleeding after traumatic injury, including hypothermia, metabolic acidosis, and dilutional coagulopathy that were recognized as primary drivers of bleeding after trauma. However,...

متن کاملBlood transfusion and coagulopathy in geriatric trauma patients
BACKGROUND Trauma resuscitation has undergone a paradigm shift with new emphasis on the early use of blood products and increased proportions of plasma and platelets. However, it is unclear how this strategy is applied or how effective it is in the elderly population. The study aim is to identify differences in transfusion practices and the coagulopathy of trauma in the elderly. METHODS Data ...
متن کاملNew insights into the acute coagulopathy in trauma patients
Abnormal coagulation parameters can be found in 25% of trauma patients with major injuries. Furthermore, trauma patients presenting with coagulopathy on admission have worse clinical outcome. Tissue trauma and systemic hypoperfusion appear to be the primary factors responsible for the development of acute traumatic coagulopathy immediately after injury. As a result of overt activation of the pr...
متن کاملCoagulopathy in Head Trauma – as Complicated as Ever
Many years ago, while I was a Chief Resident in my neurological surgery training program, I solicited the aid of my Department Chairman to have the Emergency Department add coagulation testing to the routine labs acquired on head trauma patients. Believe it or not, at that time, despite knowledge that coagulopathy frequently complicates traumatic brain injury (TBI), such testing was usually not...
